Erectile dysfunction is diagnosed with a private conversation, a focused examination and a few blood tests. Specialist tests such as penile Doppler ultrasound are needed only in specific cases. This order follows the European Association of Urology (EAU) 2025 guidelines.

Medical and sexual history
The doctor asks when the problem started, whether morning erections happen, about diabetes, blood pressure, medicines, smoking, stress and the relationship. A validated questionnaire such as the International Index of Erectile Function (IIEF) may be used to grade it and follow progress.
Physical examination
A focused examination of the genitals looks for curvature, scarring or testicular changes, with a blood pressure check and signs of low hormones.
Basic blood tests
- Blood sugar (fasting or HbA1c) to look for diabetes.
- Lipid profile to estimate the risk of narrowed arteries.
- Total testosterone: in the morning between 7 and 11 am, fasting, repeated at least once if below 12 nmol/L.
When are specialist tests needed?
For example: erectile dysfunction present from the start, young men after pelvic or perineal injury, penile curvature that may need surgery, complex hormonal or psychological problems, or at the patient’s request.
- Penile Doppler ultrasound to assess blood flow in the penis.
- Nocturnal erection testing during sleep.
- Specialised hormone tests when needed.
- Specialised psychosexual assessment.
Why your heart matters
Guidelines regard erectile dysfunction as a possible warning sign of cardiovascular disease, not only a quality-of-life issue, so your doctor may assess your heart risk factors or refer you.

Frequently asked questions
Do I need a penile Doppler scan for erectile dysfunction?
Usually not. History, examination and basic blood tests are enough for most men; Doppler and other specialist tests are kept for specific situations.
Do I need to fast for a testosterone test?
Yes. Total testosterone is measured in the morning between 7 and 11 am while fasting, and repeated if the result is low.
Why does the doctor ask about my heart?
Erectile dysfunction can be an early warning sign of cardiovascular disease, so finding and treating it early benefits your overall health.
