Erectile dysfunction and premature ejaculation do not weaken sperm by themselves. Pregnancy needs two things: ejaculation inside the vagina and healthy semen. Erectile dysfunction delays pregnancy only if it prevents penetration, and premature ejaculation only if ejaculation happens before penetration. Both are treatable.

Does erectile dysfunction prevent pregnancy?
If the erection is firm enough for penetration and ejaculation inside the vagina, erectile dysfunction does not prevent pregnancy. If it prevents full intercourse, treatment usually solves the problem, and tablets are the first treatment for most men. Read about erectile dysfunction treatment.
Does premature ejaculation prevent pregnancy?
No, as long as ejaculation happens after penetration, even if quickly. In some men ejaculation happens before or at penetration; then pregnancy may be delayed and premature ejaculation treatment helps.
Ejaculation problems that can affect pregnancy
- Delayed ejaculation or no ejaculation during intercourse.
- Retrograde ejaculation, when semen goes back into the bladder instead of coming out; it can happen with diabetes or after some operations or medicines.
Causes shared by erectile dysfunction and delayed pregnancy
Some causes affect both erections and fertility, such as diabetes, obesity, smoking and low testosterone. Importantly, taking testosterone or anabolic steroids reduces sperm production, so do not use them for erectile problems if you are trying for a baby. Read about low testosterone.
When should you have a fertility assessment?
- If pregnancy has not happened after a year of regular unprotected intercourse, according to the European Association of Urology (EAU) 2025 guidelines.
- Earlier if full intercourse is not possible or ejaculation does not happen.
- The man’s assessment usually starts with a semen analysis.

Frequently asked questions
Does premature ejaculation prevent pregnancy?
No, as long as ejaculation happens inside the vagina. Pregnancy depends on sperm reaching the egg and sperm quality, not on how long intercourse lasts. If ejaculation always happens before penetration, pregnancy may be delayed, and this is treatable.
Do ED tablets affect getting pregnant?
The tablets help penetration and ejaculation inside the vagina, which is what pregnancy needs. Your doctor chooses the right medicine after reviewing your health and medicines.
When should we have fertility tests?
If pregnancy has not happened after a year of regular unprotected intercourse, or earlier if full intercourse is not possible because of erectile or ejaculation problems.
