Erectile dysfunction and premature ejaculation often occur together. In many cases erectile dysfunction is the cause: fear of losing the erection makes a man rush to ejaculate without meaning to. That is why the European Association of Urology (EAU) 2025 guidelines advise treating erectile dysfunction first when both are present.
Medically reviewed by Prof. Dr. Samir Ahmed Al-Samarrai, consultant urologist. Last reviewed:
How are they linked?
- Anxiety about losing the erection pushes a man to finish quickly.
- Extra stimulation to get an erection can bring ejaculation closer.
- Anxiety and stress worsen both.
- Prostatitis can cause acquired premature ejaculation.
How does the doctor tell them apart?
The doctor asks which started first, whether premature ejaculation has been present since the start of sexual life or appeared later, and whether the erection fades before or after ejaculation. If premature ejaculation appeared after erectile problems, it is most likely acquired because of them. Basic erectile dysfunction tests are requested; read about erectile dysfunction tests.
How are they treated?
- Treat erectile dysfunction first, usually with tablets, lifestyle change and treating the cause. Ejaculation often improves once a man feels confident about his erection.
- Treat premature ejaculation if it continues, for example with dapoxetine or a local spray, as the doctor advises. Read about premature ejaculation treatment.
- Behavioural and psychological therapy to ease anxiety, helpful alongside medicines.
A warning about combined tablets
Many tablets are sold as treating both problems at once. Do not buy tablets of unknown origin: they may contain undeclared substances and unknown doses, and some are dangerous with heart medicines. Read about the risk of natural products.

Frequently asked questions
Can erectile dysfunction cause premature ejaculation?
It can. A man who fears losing his erection may rush to ejaculate without meaning to, so guidelines list erectile dysfunction as a cause of acquired premature ejaculation and advise treating it first.
Is there one medicine for both problems?
The doctor plans treatment for each case and usually starts with the erectile dysfunction. Buying combined tablets of unknown origin sold for both problems is not advised.
Does losing the erection after ejaculation mean erectile dysfunction?
No. Losing the erection after ejaculation is normal, followed by a period when a new erection does not come easily; this period gets longer with age.
