Many young men search for an enlarged prostate or prostate congestion when they have burning, frequent urination or pelvic pain. But benign enlargement is uncommon before 40, and symptoms in young men usually have other, more likely causes, most of them treatable once correctly diagnosed.

Causes of urinary symptoms in young men
| Cause | Distinguishing features |
|---|---|
| Chronic pelvic pain syndrome (non-bacterial prostatitis) | Pain in the perineum, testicles, lower abdomen or on ejaculation for 3 months or more |
| Bacterial prostatitis | Fever and shivering when acute, or repeated urinary infections when chronic |
| Sexually transmitted infections or urethritis | Discharge or burning in the urethra |
| Urethral stricture | A very weak stream, sometimes after injury, catheter or past infection |
| Overactive bladder | Urgency and frequency without pain or infection |
| Bladder neck obstruction or poorly coordinated voiding | Difficulty starting and a weak stream in a young man without enlargement |

What about early prostate enlargement?
Enlargement can start in some men in their forties, especially with obesity, diabetes and inactivity. Before blaming it for a young man’s symptoms, the doctor rules out other causes.
How is it diagnosed?
- A careful history of symptoms, their duration, sexual history and medicines.
- Urine test and culture, and tests for sexually transmitted infections when needed.
- Physical examination.
- Urine flow test and residual urine by ultrasound.
- The chronic pelvic pain symptom score (NIH-CPSI) when suspected.
- Cystoscopy or urethral imaging if a stricture is suspected.
PSA is not routinely requested for every young man; it can rise with inflammation and cause needless worry.
Common myths
- “Masturbation causes prostate congestion”: there is no scientific evidence for this.
- “All pelvic pain needs antibiotics”: antibiotics help bacterial prostatitis only; repeating them without evidence of infection does not help and can cause harm.
- “Congestion leads to cancer”: there is no evidence that inflammation or congestion causes prostate cancer.

Frequently asked questions
Do young men get an enlarged prostate?
Benign prostatic enlargement is uncommon before 40. Urinary symptoms in young men usually have another cause, such as prostatitis, a urethral stricture or an overactive bladder.
What is “prostate congestion” in young men?
It is a popular term rather than a diagnosis, usually describing pelvic pain and discomfort on urination or after ejaculation. The most common cause in young men is chronic pelvic pain syndrome.
Does masturbation or infrequent sex cause prostate congestion?
There is no scientific evidence that masturbation or infrequent sex causes prostate disease, and worrying about it can make symptoms worse.
Does prostatitis in young men affect fertility?
Bacterial prostatitis can temporarily affect semen in some cases and is treatable. If conception is delayed, a semen analysis assesses the situation.
When should a young man see a urologist?
With urinary symptoms lasting more than a few weeks, pelvic or testicular pain, blood in the urine or semen, fever with burning urination, or a urethral discharge.
